Conference Abstract

Background: Our young adult diabetes clinic serves a socio-economically deprived population where prevalent mental health challenges significantly hinder patient engagement and self-management, leading to poorer clinical outcomes. Recognising the strong link between psychological distress and diabetes care, we sought to address this critical issue.
Method(s): We integrated routine mental health screening using the validated CORE-10 questionnaire, administered prior to clinic appointments to identify early psychological concerns. In collaboration with a dedicated psychologist, we developed a streamlined referral pathway, ensuring timely access to specialised mental health support for patients in need.
Result(s): An audit of 134 patients from our Young Adult Diabetes Service revealed that 28% (38 patients) presented with moderate-to-severe mental health scores, necessitating referral to psychological services. These findings underscore the essential role of mental health screening in diabetes management, particularly in socio-economically disadvantaged areas where psychological distress is prevalent.
Conclusion(s): By embedding mental health screening into routine diabetes care, we have established a more comprehensive, patient-centred model that addresses both physical and psychological needs. Our initiative highlights the critical importance of integrating mental health support within diabetes services, leading to improved patient wellbeing and more effective clinical outcomes. This model demonstrates the necessity of a holistic approach to diabetes care, emphasising the intersection of mental and physical health to optimise long-term management and support young adults facing the dual challenges of diabetes and mental health concerns.
